Home Care Services Professional services, support and individual healthcare in the home setting.  A Case Manager coordinates the Home Care services as ordered by the physician. The Home Care nurse maintains continuity of care between the hospital setting, clinics, physicians, and other health care resources. Clients and their families are helped to build knowledge and skills.
