Home Care

Redwood Area Hospital Home Care services has been a trusted source of personal healthcare in your home for 25 years.

Our Home Care team meets with clients in their own space and works with them to help make their home the best place for their health.  For some clients the goal may be wound care or rehabilitation after surgery. Others simply need help with medications or homemaking.

We work with each client and their physician to create an individualized care plan. A Home Care nurse maintains continuity of care between the hospital setting, clinics, physicians, and other health care resources.

It is important to us that the client and family actively participate in developing their plan of care. We strive to provide education and resources to help aid in recovery and enhance the client’s ability to self manage their health conditions.

Free visit

Take advantage of one in-home evaluation visit by a registered nurse to discuss care needs and payment options. There is no charge for this visit.

Staff
The Home Care staff are hospital employees with background checks and are licensed, certified and insured.  Consultations with other hospital departments such as Diabetes, Cancer Care and Respiratory Therapy are also utilized.

Aide Services

• Activities of daily living such as bathing, dressing, grooming, transferring, ambulation, eating, special meal considerations, assistance with taking medications.
• Exercises to maintain or improve independence.
• Flexible scheduling to meet your individual needs.

Homemaker Services

• Meal preparation
• Light housekeeping
• Laundry

Medical Social Services
Assistance with financial applications, family counseling and individual counseling.

Medication set up

Medication caddy filled with your medications and your prescriptions are called into your pharmacy.

Caddies may be filled weekly, every two weeks or monthly.

Monday through Friday 8:00am-4:30pm

Admissions
Admission to the Home Care Service can be arranged for any day including weekends and evenings.

Nurse On-call
A nurse is on call 24-hours daily to answer questions by phone or visit in the home if necessary.

We can work with you to plan payment for Home Care services through a variety of options, including:
